With all breast implant placement methods, an incision is made that allows Dr. Pfeifer access to the breast, and the implant is centered below the nipple. The incisions are then closed with special sutures. Like most of the cosmetic surgeries we perform, breast augmentation is done under general anesthesia while local anesthesia with IV sedation is an option available in certain cases. The average time of surgery is 1 hour. After Surgery After surgery, gauze dressing will cover the incisions and the patient is given a special bra to wear during the recovery period. Swelling, bruising, and discomfort are normal after surgery, but will vary depending on the patient and the implant placement method chosen. The recovery time is generally 1-2 weeks. ENDOSCOPIC BREAST AUGMENTATION There are two popular techniques in breast augmentation that can greatly minimize the appearance of scars and they both utilize a surgical tool called an endoscope. An endoscope is a very small instrument that can be introduced into the body to view underlying tissues and parts. The two main endoscopic breast augmentation methods are the following: TUBA – transumbilical breast augmentation Transaxillary – an underarm breast augmentation incision Breasts TUBA Breast Augmentation The transumbilical method involves an incision in the navel. For this reason, some have decided to call the procedure “belly button breast augmentation.” Because the surgeon does not have direct access to the breasts with this method, an endoscope is necessary. After making the incision near the navel, Dr. Pfeifer creates a tunnel through the abdominal tissues to the breasts. The implants are then placed in the breast pocket, in a submuscular or subglandular position. After placement, the breast implants are filled to a specified volume with saline. The transumbilical breast augmentation technique requires a skilled breast surgeon, but when it is performed correctly the patient can obtain beautiful results with scars that are undetectable. Transaxillary Breast Augmentation The transaxillary technique uses an incision in the natural skin folds of the armpit, which allows scars to be well hidden and far away from the breast. Like the TUBA method, the distance to the breast is a factor to consider, but plastic surgeons can perform the transaxillary technique with or without the use of an endoscope. The incision is made in the underarm and Dr. Pfeifer performs a dissection to obtain a view of the breast pocket. The implant is then placed in one of the three possible placement methods – submuscular, subglandular, or partial submuscular – and filled with saline to a specified volume. Silicone breast implants may also be used, but require a slightly longer incision. After surgery, the scar is small (2-5 cm) and well hidden. Endoscopic Disadvantages The main drawback of these procedures is the possibility of revision surgery in the future. If a problem occurs such as capsular contracture, the patient will require a conventional incision near the breast. Some plastic surgeons choose not to perform endoscopic breast augmentation or TUBA breast augmentation because it does not allow direct access to the breasts during surgery.

  Who Can Benefit From A Breast Lift? The goals of breast lift surgery are slightly different than those of breast augmentation, mainly because the breast lift cannot enhance or restore breast fullness. Those who exhibit the following conditions may benefit from breast lift surgery: Excess breast sagging Lack of breast firmness Lack of skin elasticity Nipple ptosis About The Breast Lift Surgery To correct sagging and reposition the nipple, there are a variety of surgical techniques that can be utilized. All breast lift surgeries require an incision around the nipple, either partial or circumferential. A vertical incision is also necessary in many cases, especially when the nipple needs to be elevated. After the incisions, a certain amount of skin and glandular tissue is removed and sutured to produce a more appealing breast position and shape. After Surgery After the mastopexy, the incisions and sutures will be covered with gauze or tape. A special bra is provided that must be worn during the recovery period. It is normal to experience some bruising, swelling, and discomfort during this time, but pain is usually well controlled by medication. The scars that result from breast lift surgery are noticeable at first, but flatten out and fade over time. Nonetheless, they remain permanent and that is something patients should consider before surgery. There are other possible risks such as loss of nipple sensation. Breast Lift With Augmentation A breast lift with augmentation, otherwise known as augmentation mastopexy, corrects breast sagging and increases breast size in a single surgical procedure or two staged surgical procedures. Augmentation mastopexy generally involves the placement of a saline or silicone breast implant in either the submuscular or subglandular position, followed by breast lift surgery using incisions around the nipple or vertically down the breast. Who Can Benefit From Augmentation Mastopexy? Women who exhibit a loss or absence of breast volume, along with significant breast sagging will benefit most from augmentation mastopexy. Here is a very generalized set of rules to explain when the breast lift & augmentation is appropriate: If breast volume and size is sufficient, a breast lift is usually all that is needed. If breast position is sufficient, but volume and size are lacking, a breast augmentation is needed. When breast size, fullness, and position are all deficient, augmentation mastopexy may be necessary to improve the appearance of the breasts. The Surgery All breast lift surgeries require an incision around the nipple, either partial or circumferential. A vertical incision is also necessary in many cases, especially when the nipple needs to be elevated. After the incisions, a certain amount of skin and glandular tissue is removed and sutured to produce a more appealing breast position and shape. Breast implants can be placed in multiple ways. With all breast implant placement methods, an incision is made that allows Dr. Pfeifer access to the breast, and the implant is centered below the nipple. In augmentation mastopexy, the periareolar incision (around the nipple) is often used to place the breast implant. Average recovery time for either procedure is 1-2 weeks, but a longer recovery time may be necessary when they are combined. • Breast Reconstruction

  When Is The Best Time For Breast Reconstruction? Reconstructive surgery should be planned and discussed prior to the mastectomy with your breast surgeon and Dr. Pfeifer. Ideally, reconstructive breast surgery should be performed during or immediately after the mastectomy, when the chest wall is unaffected by radiation or scarring. Unfortunately, for various reasons, some patients don’t consider immediate breast reconstruction. Such reasons may be personal, or it may be the case that radiation in the chest is needed post-mastectomy, in which case it is usually advised to delay reconstruction until after the completion of radiation treatment. Breast Reconstruction Techniques There are two general techniques plastic surgeons utilize for breast reconstruction; one technique may use a synthetic implant, while another may use the patient’s own tissue. Reconstruction With Breast Implants When breast implants are used, a saline or silicone gel implant is placed and the new breast shape is molded around it. Surgery may require several stages and the use of a tissue expander, especially when the chest wall tissues are tight and/or flat. Reconstruction with Existing Tissue There are several breast reconstruction methods that use skin, fat, or muscle to form a “tissue flap” that helps recreate the breast. TRAM: the transverse rectus abdominis muscle flap uses abdominal tissue as a donor site. The procedure is comparable to an abdominoplasty, or “tummy tuck.” Latissimus Dorsi: this tissue flap uses tissue of the upper back as a donor site. DIEP: the deep inferior epigastric artery perforator tissue flap is comparable to the TRAM, but handles tissues of the abdominal muscles differently. SGAP: the superior gluteal artery perforator tissue flap uses the buttocks as a donor site. Recovery & Results Recovery time varies. If breast implants are used, the patient will experience some fatigue and discomfort for 1-2 weeks after surgery. With tissue flap surgeries, the recovery often takes longer. A few months after the patient has recovered from breast reconstruction, the nipple can be reconstructed.   Who Can Benefit From Liposuction? Liposuction is best suited for patients who have diet-resistant areas of excess fat. Obese patients are not considered good candidates for the procedure. Patients are usually advised to wait until their body weight has stabilized. This may require waiting 18-24 months after bariatric surgery. To undergo a body lift, patients should also be in good mental and physical health. Insurance may even cover some cases such as when the patient’s ability to walk is hindered. However, most surgeries of this type are considered elective and are therefore not covered. The post bariatric body lift is a major operation. Those who will benefit most are willing to accept scars in order to attain improved body shape and contour. The Procedure The surgery is usually done in stages, combining multiple liposuction and excision operations on the central body. The back, lower abdomen, inner and outer thighs, and buttocks are addressed in a circumferential fashion. When possible, incisions are made in natural body creases so they remain inconspicuous after surgery. The redundant skin on the body is removed or lifted and the incisions are closed with sutures. • Bio-Identical Hormone Therapy

  As we age, our hormone levels decrease. As this happens, we can experience many symptoms, which are listed below. Many patients will say to themselves, “Well, this is what it is like to get older,” and suffer in silence, but we do not have to live with these symptoms. Bio-identical hormone replacement therapy, with plant-derived hormones, can improve these symptoms. In addition to decreasing symptoms, BHRT can have a positive impact on heart health and decrease the risk of Alzheimer’s and osteoporosis. Bio-identical hormones can be beneficial for both women and men. While women are familiar with the symptoms of menopause, men can also have “manopause,” with symptoms due to low levels of testosterone. In some cases, patients need thyroid optimization as well. While most of us are familiar with bio-identical hormone therapy for women going through menopause, both men and women can experience low levels of testosterone, even in their 30s. Some of these people can benefit from bio-identical hormone replacement. For example, most women have lost ½ of their testosterone levels by age 40. Testosterone levels decline 1-1.5% per year in men after age 30. So, some men can benefit from testosterone replacement even when they are in their 30s, and women can benefit from testosterone replacement even before menopause. Of course, each person must be individually evaluated with a thorough medical history and blood tests. In addition to BHRT, patients may need supplements, including iodine and vitamin D, to optimize their health. Women with a history of breast cancer are not candidates for estradiol replacement. BHRT is the process by which hormones in the patient’s body are replaced with hormones that are molecularly identical.
