IAHD Home Health and Hospice is a women-led, Minority Driven Organization that is ran by leaders who are passionate in helping our commuity sustain longevity, an active and healthy lifestyle through quality, reliable, and profesional home health services. We specialize in Home health and Hospice. CDPAP - A special program where you can select a friend or family member to take care of you. Since November 2012, the Consumer-Directed Personal Assistance Program (CDPAP) has been included in the benefit package of Mainstream Medicaid Managed Care and Managed Long Term Care. The consumer must consult their Managed Care/MLTC Plan in order to be ensured that they can get CDPAP.
