  Nipple Reconstruction

  Nipple reconstruction is the surgical process of recreating the nipple after a mastectomy to treat breast cancer. Many patients will first undergo autologous breast reconstruction, or flap reconstruction, where the plastic surgeon transfers a flap of living tissue to the chest to create the breast mound. If flap reconstruction is not right for you, breast implants are also an option to restore the appearance of breast tissue. The final step in breast reconstruction is repairing the nipple. Nipple reconstruction is a two-part process. The first is a small procedure to create the elevated appearance of the nipple projection from a flap of the patient's own skin. After this heals, we move onto the next step, 3D tattooing. We work with specialized tattoo artists who have experience in creating realistic nipple dimensions and areola definition.

  TUG and PAP Flap Breast Reconstruction

  While DIEP is a popular breast reconstruction technique, patients who are slender or who have undergone abdominal surgery before typically do not have enough fat tissue to harvest. In such cases, transverse upper gracilis (TUG) and profunda artery perforator (PAP) flap surgeries may be recommended. Both surgeries use harvested tissue from one or both inner thighs. The major difference between the TUG and PAP flap procedures is the type of tissue harvested. While the PAP flap harvests only fatty tissue, TUG flap harvests both fatty and muscle tissue.

  Either immediately after mastectomy or at a later time, Dr. Lerman can perform reconstruction by placing implants or through autologous reconstruction (using the patient's own tissue to rebuild the breast mound) for one or both breasts.   Restoring Sensation with Nerve Grafts

  To help restore sensation, Dr. Lerman re-innervates the flap by reconnecting a severed nerve with a nerve allograft at the time of natural tissue flap surgery. By reconnecting the nerves, Dr. Lerman gives the body the best possible opportunity for the nerve to regrow and help provide sensation to the reconstructed breast. Although there will always be some numbness after mastectomy, the potential sensation return achievable after natural tissue breast reconstruction is dramatic.
